We are transforming primary care in Oregon by integrating treatment for substance use disorder, HIV, and STIs —centered on people impacted by the carceral system and served by FQHCs—through statewide collaboration, expanded access, and a shared commitment to treating addiction as a chronic disease.

GOAL 1 PROGRESS

GOAL 1: Create an evidence-based action plan for this group.

Year 1: Quarter 2 (Oct-Dec 2025)

Current Milestone:

  • In progress: Dissemination of survey to OPCA members.
GOAL 2 PROGRESS
GOAL 2: Build relationships between local county jails and their local FQHCs.

Year 1: Quarter 2 (Oct -Dec 2025)

Current Milestones:

  • In progress: FQHC call to action.
  • Identify carceral institutions and FQHC potential partnerships – identify stakeholders.
  • Stakeholder meeting between FQHC and local county jails.
  • Develop a workflow for referral/access to care from jails.
GOAL 3: Identify and advocate for a sustainable funding stream for contingency mgmt for meth and AUD in primary care.

Year 1: Quarter 2 (Oct -Dec 2025)

Current Milestones:

  • In progress: Identify CM programs throughout the state and reach out to discuss how they have funded and implemented CM with a focus on FQHCs.
  • In progress: Reach out to FQHCs in surrounding states who have implemented CM.
  • Collect data to get cost per member for a stay in detox or emergency room visits.
  • Collect data on potential cost savings of CM implemented in primary care vs. repeated detox stays or ED visits.
GOAL 4 PROGRESS

GOAL 4: Create a standard of care for PSUD at Oregon FQHCs.

Year 1: Quarter 2 (Oct – Dec 2025)

Current Milestone:

  • In progress: Define what we are looking to create SOC for, i.e.: Harm reduction, Referral, OUD/AUD/Meth UD, Care team composition (A range of experience/capacity should be reflected in SOC)
  • Technical training and support.
  • Explore funding for this work.
GOAL 5 PROGRESS
GOAL 5: All 34 Oregon FQHC will either offer LAI SUD medications or have clearly defined pathways for referral bidirectional care coordination.

Year 1: Quarter 2 (Oct -Dec 2025)

Current Milestones:

  • In progress: ID which clinics already use LAI and which do not.
  • In progress: ID what barriers there are to those who are not.
  • ID process used by clinics already using LAI.
  • Map potential referrals / community partners.
GOAL 6 PROGRESS
GOAL 6: Build provider capacity through ed related to HIV/STIs/MAT/justice-involved pop that is integrated into HCs internal ed processes.

Year 1: Quarter 2 (Oct -Dec 2025)

Current Milestones:

  • In progress: Conduct an environmental scan of existing provider tools on HIV, STIs, MAT and justice-involved care.
  • In progress: Identify key partners: CCOs, public health departments, DOC, peer-run orgs, CHC educators.
  • Develop shared goals and criteria for integrated education modules.
  • Convene advisory committee of clinicians, educators, and persons with lived experience.
  • Create a roadmap for learning integration (ie.e. CME credit, lunch and learn, onboarding).
  • Develop learning objectives and core competencies in each topic area.
