The Director of Pharmacy provides operational leadership, direct supervision, expert guidance, and performance oversight for the Pharmacy program to provide the highest level of medical and dental care, health education, and preventive services possible to patients. This position leads the expansion and sustainability of pharmacy services within Neighborhood Health Center (NHC). Pharmacy services include the primary care clinical pharmacy service and dispensing pharmacy service. The Director will split time between administrative duties and provision of patient care as part of a multidisciplinary care team.

Essential Job Duties
Clinical Pharmacy:

Provides direction and clinical oversight for NHCs clinical pharmacists and clinical pharmacy program.
Identifies and investigates opportunities for grant funding of pharmacy department initiatives.
Ensures profitability and sustainability of clinical pharmacy services program.
Partners with Director of Primary Care Operations, Director of Nursing, Director of Behavioral Health, and Medical Director of Primary Care in leading the operation of a highly functioning, clinically integrated care team within primary care.
Uses and regularly updates NHCs Primary Care Collaborative Drug Therapy Management (CDTM) protocols for disease management in collaboration with the Chief Medical Officer.
Performs duties of Principal Pharmacist of CDTM protocols per Board of Pharmacy and Board of Medicine rules.
Makes all hiring and firing decisions for assigned positions in coordination with HR.
Provides staff with the training, mentoring and resources necessary to carry out their work.
Manages and evaluates performance of assigned staff. Takes appropriate action when needed.
Performs other duties as assigned.
Participates in direct patient care with NHC as a clinical pharmacist or covering the dispensing pharmacy for 0.1-0.2 FTE of your total time.
Dispensing Pharmacy Service:

Partners with Chief Medical Officer, Chief Dental Officer, Director of Dental Operations, Director of Risk, & Director of Nursing to ensure the pharmacy department operates in compliance with all applicable state and federal rules and regulations.
Identifies and investigates opportunities for grant funding of pharmacy department initiatives.
Implements policies and procedures that ensure the dispensing pharmacy department provides optimal pharmaceutical services that meet all legal, accreditation, and certification requirements.
Provides regular quality assurance of pharmacy department programs and staff.
Develops and maintains internal P&T Committee.
Supports the expansion of pharmaceutical care services to patients.
Provides comprehensive medication review for patients with systematic assessment of each medication for appropriateness, efficacy, safety, and adherence.
Evaluates the appropriateness of medication utilization. Takes action to prevent or correct deficiencies.

Education and/or Experience
Required:

Doctoral Degree in Pharmacy (Pharm D) from an accredited institution.
Must have completed a PGY1 and PGY2 residency or possess board certification from the Board of Pharmacotherapy Specialties.
At least five years of experience of ambulatory and outpatient pharmacy practice.
At least three years of experience as a clinical pharmacist within a primary care setting.
Preferred:

At least three years of supervision of pharmacy staff across clinical and dispensing pharmacy settings is preferred.
Licensure/Certification:

Current and unrestricted Oregon Pharmacy license is required. Board Certification granted by the Board of Pharmacotherapy Specialties (BPS).
