Preparing Health Centers for social risk screening and referrals is the aim of our SDOH Workgroup. In these meetings we will share emerging strategies, disseminate the latest guidance from state authorities, and find the best ways to strengthen the safety net with community-based organizations. Health equity and population health management will be at the core of our efforts in this group.
