Value-based care (VBC) is a health care model that ties payment to improved patient outcomes, rewarding providers for delivering high-quality, patient-centered, and cost-effective care. For FQHCs, this model is especially important because it supports comprehensive care for patients with complex needs while helping health centers remain financially sustainable. As innovators in care delivery, FQHCs play an important role in advancing value-based payment and care models, offering approaches that other healthcare systems can learn from.
OPCA’s VBC programing helps health centers prepare for and succeed in this shift. Through payment reform initiatives, collaborative learning opportunities, and shared best practices, we provide tools and frameworks that strengthen care delivery, advance health equity, and improve health outcomes across Oregon’s safety net.
What We Offer
- Alternative Payment & Advanced Care Model (APCM) – A nationally recognized alternative payment methodology advancing payment reform and care transformation since 2010. APCM is now implemented in 20 FQHCs and two Rural Health Clinics across Oregon.
- APCM Learning Forums (formerly ACLC) – A webinar and workshop-based learning series that supports care model transformation through themed learning cohorts.
- The Oregon Network of Community Health Centers (The Network) – A self-governing, clinically integrated network (CIN) established in 2020 by Oregon’s FQHCs and Look-Alikes. The Network operates as a learning collaborative to develop, share, and standardize best practices across health centers. Its priorities include advancing population health outcomes, improving healthcare quality, reducing total cost of care, and promoting health equity.
- Value Based Care Resource Repository – A centralized library of resources, tools, and best practices to help health centers navigate value-based care. The repository is available to FQHC members with Member Portal access.
