Title:Program Manager - Complex Care Management

Description:Northwest Human Services is a non-profit leader in providing advocacy, quality healthcare and social services in Marion and Polk counties since 1970. We’re looking for a highly experienced leader to join our team as the Program Manager of Complex Care Management.

As our Program Manager of Complex Care Management, you will have the opportunity to build an agency-wide care management strategy from the ground up! Using data and evidenced-based practices to analyze our care management model, you will develop and implement a workflow that supports patient experience improvements, promotes positive clinical/quality outcomes, and reduces health care costs. This model will be applied across all programs to ensure consistent service delivery.

Additionally, this role will supervise our case managers, behavioral health providers, and community health workers in both our Medical Clinics and Transitional Programs with a strong emphasis on integrating efforts across departments, multidisciplinary teams, and programs. A strong sense of collaboration and communication with other managers is required, as you will provide supervision under a Matrix model.


This position’s work schedule is day shift, Monday – Friday.  The primary work location will be in downtown Salem, Oregon at one of our Transitional Programs, or in West Salem at the West Salem Medical Clinic.  Travel is expected due to direct reports being dispersed at multiple locations, including our Total Health Community Clinic located in Monmouth, Oregon.
* Licensed Clinical Social Worker (LCSW), or other licensed healthcare or behavioral health care clinician with current appropriate state licensure -OR-
* Master's Degree in a Behavioral Health field, plus at least 2 years care management program leadership experience -OR-
* Registered Nurse (RN)

* 2 years of clinical experience
* 2 years of program development experience
* 2 years of supervisory or management experience
* Ability to develop, review, and evaluate workflow, utilization, and care management reports
* Proficiency in data analysis and quality metrics.
* Experience with vulnerable populations, coupled with a strong desire to serve the underserved.
* Desire and ability to promote equity and inclusion.
* Proven ability to maintain a positive attitude and collaborative work style
* Ability to manage relationships in order to progress the goals of the program and team.
Solid knowledge of community services, providers, vendors, and facilities available to support patients and clients.
