CCC’s Old Town Clinic is seeking a Summit Care Coordinator!

Old Town Clinic provides compassionate health care to people who might not otherwise get care, treating them with dignity and respect no matter how complex their care. Located in the heart of the Burnside neighborhood, patients can often get all the care they need in a single location.

The Summit team is not an acute care team, but a highly trained, integrated primary care team supporting patients with advanced illness. The Summit Care Coordinator is a Medical Assistant that also provides health coach services.

Schedule: Monday-Friday 8:45am-5:15pm

Duties will include:

With the PCP manage a panel of approximately 100 patients.
Serve as main point of contact for patients when they need to interact with care team.
Lead pre, intake, follow up and post visits for all patients attending medical appointments with the Summit team.
Advise patients on goal setting and provides action plans for managing their disease.
Collect information regarding clients’ health history, current health behaviors, and bio/psycho/social needs through a combination of direct interviewing, records review, and consultation with outside sources (after obtaining written consent).
Outreach as needed by providing home visits with other team members, accompany patients to specialist visits outside of Old Town Clinic and attempt to contact low-engagement patients in the community.
Participate in recruitment of patients to the Summit team.
Participates in tasks such as minor procedures set up, call management and appointment scheduling.

Completion of a Certificate-level Medical Assistant program accredited by the Commission on Accreditation of Allied Health Educational programs (CAAHEP).
Current MA certification from a nationally recognized program (such as AAMA orNCCT), or must obtain certification within 90 days of hire.
At least one (1) year recent, verifiable medical assistant experience within a health care setting preferred
Must have current CPR certification at time of hire
Must possess a current drivers license, pass a DMV background check and be designated an “acceptable” driver as outlined in Central City Concern’s Fleet Safety Policy. Must pass an initial drivers training within 60 days of being an approved driver and continued recertification training.
Must pass a pre-employment drug screen, TB test, and background check.
