CCC’s Blackburn Center is seeking a Mental Health Counselor, QMHP!
Blackburn Center is a new six-floor building in East Portland, Oregon (clinic and housing) that will give vulnerable people access to integrated housing, employment and health services, and provide support they need to stabilize and rebuild their lives.
The Mental Health Counselor is responsible for the delivery of care to the population assigned to the behavioral health home, ensuring care that is accountable, comprehensive, proactive, well-coordinated and client-centered.
Duties will include:
Collect information regarding clients’ history through a combination of interviewing, records review, and consultation, after obtaining written consent.
Conduct and document Mental Health Assessments, including mental status exam, DSM diagnosis, and clinical formulation.
Develop care plan with client with assistance & input from all of the clients care team providers
Conduct educational and therapeutic groups for clients, using approved curriculum.
Coordinate services with all members of client’s support network.
Master's degree in a human services field and one year experience in mental health. Experience with severe and persistent mental illness (SPMI) preferred.
At minimum, must meet state guidelines for QMHP and be licensed/certified with appropriate board.
Gather histories of mental, emotional and physical disabilities, alcohol and drug use, past mental health services and criminal justice contacts;
Current CPR certification at time of hire
Must pass a pre-employment drug screen, TB Test, and background check. This includes clearance by the DHS Background Check Unit.
