Description: | Rogue Community Health is looking for a RN Nurse Case Manager for our clinics who is not only committed to exceptional patient care, but equally interested in supporting staff within a cohesive team environment. This position will ensure the efficient and effective flow of patients during scheduled clinics. Within the RN scope of practice, utilize the nursing process to provide patient care and education which will focus on patient engagement and behavioral change, treatment compliance, and health risk factor reduction. This position will work collaboratively through joint planning and problem solving with clinic staff and improve the management and stability of medically complex patients resulting in reduced ER visits, hospitalizations, treatment-related complications, and improved patient quality of life. Enjoy stable hours, competitive pay, and benefits, and an $1,500.00 signing bonus after six months of continued employment. Bonus amount is prorated based on FTE.
