Description: | Family Nurse Practitioner Columbia River Health (CRH) of Boardman Oregon is looking to hire a Family Nurse Practitioner for a family practice position at our clinic. CRH is located in Central Eastern Oregon along the Columbia River near Hermiston Oregon. Our clinic is small Federally Qualified Health Center (FQHC) serving the needs of rural residents of Boardman. We have a particular interest in serving the needs of our migrant and agricultural patients. CRH is a Patient Centered Primary Care Clinic with integrated Behavioral Health and substance abuse treatment. The clinic is staffed with two Physicians, three Physician Assistants, two Pharmacists, one LCSW and one MSW. We have a full complement of support staff including care management, community health workers and RN support staff. Our clinic has a full complement of bilingual Spanish staff with three clinicians fluent in Spanish too. The clinic provides full primary care with two procedure rooms and an onsite X-ray machine. Please be advised that a condition of employment with Columbia River Health is dependent upon your ability to provide proof of your eligibility to work in the United States, and results of your criminal background check, pre-employment drug screening, references and results of a TB blood test. This position requires completion of the COVID 19 vaccination series. General Duties and Responsibilities The Family Nurse Practitioner works as part of the clinic team performing a variety of direct patient care functions including taking history and physicals, diagnostic procedures and formulating care plans. The Family Nurse Practitioner works with the clinic personnel to help manage the needs of their panel of patients. This would include coordination of care, managing hospital / ER follow up and helping patients with social needs. The Nurse Practitioner enjoys practice autonomy but has access to consultation with the clinic physicians at any time to assist in patient care. The Family Nurse Practitioner operates under the supervision of the Chief Medical Officer. Specific Duties and Responsibilities 1. Assess and institute appropriate care for patient from newborns to geriatrics within established guidelines reflecting the standard practices of primary care 2. Prescribes and adjust medications according to therapeutic goals and explains treatment to patients, families and other clinic personnel 3. Performs other necessary duties as required by CRH to meet the goal of providing primary health care services 4. Orders routine and diagnostic lab and x-ray studies according to standard medical practice 5. Diagnoses acute and chronic illness on the basis of clinical findings, laboratory and x-ray reports and the results of other diagnostic procedures according to approved standards 6. Interprets x-ray images obtained in the clinic for a “first look” with a subsequent report generated by a radiologist interpretation 7. Interprets ECG images obtained in the clinic for a “first look” with a subsequent over read by a physician 8. Formulates, initiates and monitors patient care plans for patients assigned to the provider, using the prevailing practices and/or consultation with a supervising physician when necessary 9. Refers patients requiring hospitalization or assessment and management of conditions to supervising physicians or appropriate specialist 10. Renders urgent care and treatment for urgent or emergent cases that may present to the clinic under appropriate standards and within the scope of the practitioner and capabilities of the clinic 11. Maintains all patient records to comply with required standards, reviews records regularly and obtains physician counter signatures where necessary 12. Attends all medical staff meetings and participates in CRH Q/A program 13. Required to develop proficiency in using OCHIN EPIC, our EHR platform, for providing and documentation of care 14. Take phone only call for the clinic afterhours when assigned to do so as part of providing continuous primary care 15. Support the clinic in providing services and care for patients with substance abuse disorder or mental health conditions within the scope and experience of the practitioner 16. Works cooperatively and supportively with all other staff Additional practice options 1. Participation in substance abuse treatment program after meeting additional qualifications 2. Nexplanon / IUD placement, removal and management after meeting additional qualifications 3. Colposcopy after meeting additional qualifications Qualifications 1. Graduation from an acceptable type A Nurse Practitioner program recognized by the Board of Medicine of the National Academy of Sciences or from a program otherwise designated for Nurse Practitioners which has been approved by both the State Medical Society and the American Medical Association 2. Current Oregon license without restrictions 3. Current DEA registration with an expectation to obtain an X-waiver within 12 months of hire 4. Must be eligible to be credentialed and contracted through Medicare, Medicaid, and Commercial Payers 5. Must be able to meet deeming requirements for FTCA and gap malpractice coverage 6. Able to participate without restrictions with Medicare and Medicaid 7. The ability to relate with effectiveness to patients and providers 8. Carry a current BCLS card or obtain within 180 days of start 9. Must demonstrate proficiency in writing clearly, communicating and speaking English 10. Providers wishing to conduct exams in Spanish or any foreign language must pass a proficiency exam 11. Must be able to appropriately interact with a translator for examinations occurring in languages practitioner is not proficient in 12. Able to demonstrate basic computer proficiency in using Windows, Microsoft Office and security IT processes Physical Requirements 1. Must be able to lift and carry up to 50lbs 2. Required to have normal vision and hearing with or without correction 3. Be able to tolerate continuous standing, walking, stooping, bending and kneeling 4. Be able to perform fine motor tasks 5. Must have the dexterity and manual ability to write and type on a keyboard at a rate of at least 25 words per minute Reporting Relationship The Nurse Practitioner reports to, is supervised by, and is evaluated by the Chief Medical Officer. The Nurse Practitioner may be asked to supervise a Clinical Nurse or other members of the primary care team. |
