Essential Functions/Major Responsibilities:
JOB FUNCTIONS

ESSENTIAL FUNCTIONS are critical and fundamental responsibilities of the position of all CHN2s. The incumbent must be able to perform these functions with or without reasonable accommodation. Community Health Nurses will follow the six standards of practice:

Assessment – Collects comprehensive data related to the health of the population served. Documents assessment data in terms that are understandable to all involved in the care process.
Diagnosis and Priorities – Analysis of the assessment data to derive a nursing diagnosis and priorities. Document diagnoses or concerns in a manner that facilitates the population’s involvement in developing a plan of care.
Outcomes identification – Based on the diagnosis the CHN will identify outcomes that are culturally relevant, health literate, based in evidence and attainable through local resources and policies. Document expected outcomes and measurable objectives.
Planning – Develop a plan that is client-centered and reflects best practices by identifying strategies, action plans and alternatives to meeting expected outcomes. Document the plan using health literate and culturally competent language that can be understood by care team and population being served.
Implementation – The CHN oversees the implementation of the plan through treatment, case management, referrals to community partners, collaboration with community resources, health promotion and education. Documents activities and status of care coordination.
Evaluation – Applies on-going assessment data to revise plans, interventions and activities as appropriate for ultimate health outcomes for the population. Documents changes to enhance effectiveness of interventions.
For Primary Care and School-based Health Center CHN2 will perform additional job responsibilities:

Applies theories and principles of primary care nursing practices. This includes services in the two VA Clinics.
Consistently sets appropriate priorities in providing quality patient care that is organized and time efficient. Utilizes staff feedback in patient care. Maintains and ensures confidentiality of patient’s conditions, records, and other data on a consistent basis.
Perform direct care activities by administering medications, observing, and documenting patient responses and side effects. Administers interventions according to established standing orders and protocols.
Provide an environment that demonstrates caring, compassion, and respect for all patients and families. Creates and maintains a high-performance clinic environment characterized by positive role model, leadership, and a strong team orientation.
Communicate, as necessary, with other clinic and departmental personnel to coordinate efforts, resolve operational problems, and provide high quality, comprehensive patient care.
Meets training requirements and practices according to HRSA, HHS and VA guidelines
Participates in on-going quality improvement and reports from data systems to guide and improve practice.
Establishes professional rapport with clients, community, and health care partners to promote health.
OTHER JOB FUNCTIONS are those duties that are performed either very infrequently or could be performed by others without altering the underlying reason the job exists:

Attend and participates in team meetings, trainings, and activities to further the health of the community.
Typical Qualifications:
MINIMUM QUALIFICATIONS

Minimum Education Level

Associates and/or Professional Certification

Specifics: Associate degree in Nursing from an accredited school of nursing.

Minimum Experience Level

Two- Three years

Specifics: Three years of nursing experience.

Substitution Note: Practicums in Nursing programs and lived experience may substitute for years of experience.

Driver’s License

Oregon Driver’s License required.

Specifics: A valid, insurable State of Oregon driver’s license at the time of appointment.

Other Licenses

Professional License required.

Specifics: Must possess a valid license to practice in the State of Oregon as a Registered Nurse.

Desirable Qualifications: Bachelor of Science degree in Nursing (BSN) from an accredited school of nursing with one year of nursing experience, preferably in a primary care position. Bilingual and/or bicultural competencies for the Latino population are preferred.

KNOWLEDGE, SKILLS AND/OR ABILITIES

Maintain the confidentiality of privileged information in accordance with HIPAA and Lincoln County policies.
Maintain effective working relationships with staff, supervisors, physicians, other agencies and the public; create a positive work environment that promotes productivity, mentoring, teamwork and cooperation; elicit and consider differing viewpoints when analyzing issues; recognize the accomplishments of team members.
Convey information, ideas, and facts both orally and in writing to supervisors, colleagues, and individuals, inside and outside the county, using language and a format the audience will best understand.
Utilize computers and other technology to communicate in written and verbal forms (e.g., word processing, texts, emails, spreadsheets, department-specific software).
Effectively relate well with others, including supervisors, colleagues, and individuals inside and outside the county. Exhibit a professional manner in dealing with others and working to maintain constructive working relationships.

Job Conditions and Physical Demands:
WORKING CONDITIONS

Job Conditions: Normal working hours. Exposure to potentially hazardous conditions due to frequent drawing of blood.

Work Location: Lincoln County

Travel: Some driving between clinics may be required.
