Work in a community setting, providing quality nursing care in the Lincoln Community Health Center Veterans Administration (VA) program. Your job also involves . . . Providing direct patient care as the RN for the Office Based Addiction Treatment (OBAT), School Based Health Center and VA as assigned. Promotes health through education, care coordination and health maintenance. Essential Functions/Major Responsibilities: ESSENTIAL FUNCTIONS are critical and fundamental responsibilities of the position of all CHN2s. The incumbent must be able to perform these functions with or without reasonable accommodation. Community Health Nurses will follow the six standards of practice: Assessment – Collects comprehensive data related to the health of the population served. Documents assessment data in terms that are understandable to all involved in the care process. Applies theories and principles of primary care nursing practices. This includes services in the two VA Clinics. Attend and participates in team meetings, trainings, and activities to further the health of the community. Minimum Education Level Associates and/or Professional Certification Specifics: Associate degree in Nursing from an accredited school of nursing. Minimum Experience Level Two- Three years Specifics: Three years of nursing experience. Substitution Note: Practicums in Nursing programs and lived experience may substitute for years of experience. Driver’s License Oregon Driver’s License required. Specifics: A valid, insurable State of Oregon driver’s license at the time of appointment. Other Licenses Professional License required. Specifics: Must possess a valid license to practice in the State of Oregon as a Registered Nurse. Desirable Qualifications: Bachelor of Science degree in Nursing (BSN) from an accredited school of nursing with one year of nursing experience, preferably in a primary care position. Bilingual and/or bicultural competencies for the Latino population are preferred. KNOWLEDGE, SKILLS AND/OR ABILITIES To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The required knowledge, skills and/or abilities are listed below: Knowledge of: The principles, practices, methods, and procedures of nursing as applied to community and public health. Skill or ability to: Build therapeutic relationships and maintain therapeutic boundaries. Job Conditions and Physical Demands: Job Conditions: Normal working hours. Exposure to potentially hazardous conditions due to frequent drawing of blood. Work Location: Lincoln County Travel: Some driving between clinics may be required. PHYSICAL DEMANDS The physical demands are typical of those that must be met by an employee to successfully perform the essential functions of this job. ADA COMPLIANCE: Physical capability to handle high mobility demands and complex personal interactions.
