Opportunity

This position works with a designated clinical care team to provide short-term case management and patient support in an effort to identify social determinants of health. The CHW is responsible for addressing and removing these barriers to improve patient care and health outcomes. This position recruits for and teaches assigned patient education opportunities. This position may be asked to provide periodic staffing support for outreach events. Bilingual applicant and Traditional Health Worker Certification strongly preferred.

 

Responsibilities

Screen patients for social determinants of health (barriers) and assist patients and care team members in the development of care plans and patient goal setting.
Train to be certified Patient Educators (Walk with Ease, Living Well Series, etc.)
Lead patient education classes as determined by program and patient needs.
Build and maintain positive working relationships with the patients, care team members, healthcare entities, social service agency representatives, while representing Mosaic Community Health in and out of clinic.

Skills & Knowledge

Excellent communication and interpersonal skills. Strong, proven computer proficiency. Knowledge of community social service agencies and programs. Fluency in Spanish preferred.
