Registered Nurses work in clinical, community, and home settings to provide a broad range of nursing services aimed at disease prevention, treatment, and health policy implementation. They apply critical thinking and nursing process skills to screen, assess, plan, deliver and evaluate health interventions for individuals and populations. This position will include triage to assess clients’ health status for immediate care, clinical appointment, education for care at home, or referrals to resources. Demonstrate knowledge-based care to assigned clients and programs in accordance with standards of practice, nursing practice act, Oregon Administrative Rules, County and department policies and procedures. May provide specialized services within specified programs and at multiple sites according to scope, license, and program standards.
This position will work at both the East Linn Health Center, located in Lebanon, and the Sweet Home Health Center. These are full-spectrum Primary Care Clinics focused on team-based patient care for community members of all ages and stages of life.

This position may be filled as a Health Care Professional 2 or a Health Care Professional 3 level depending on experience, education, and specialized certifications.

The following minimum qualifications are required for this position:
Health Care Professional 2
Associate’s Degree in Nursing (RN); AND
3 years of nursing experience
Health Care Professional 3
Bachelor’s of Science in Nursing (BSN); AND
1 year of nursing experience
Equivalency will be considered for RN and 4 years of nursing experience
An equivalent combination of education and experience may be accepted.
Currently licensed in Oregon as a Registered Nurse
Successfully pass a criminal background check
Valid Oregon Driver’s License
Triage experience with adults and pediatrics
Experience working with a care team and scheduling staff
Experience in electronic medical records

Annual salary range of:
Health Care Professional 2: $61,225 – 88,547
Health Care Professional 3: $64,289 – $92,978
These positions are eligible for up to two salary raises within your first year. An Increase may be given at the end of the 6-month probationary period, depending on starting salary, and at the 1-year anniversary, as long as your performance is in good standing! Annual performance-based increases are given until the maximum of the salary range is reached.
