- Highlight TA efforts on food insecurity as a social determinant of health, including relevant research and a model to implement nutrition interventions in the primary care setting
- Identify health center workflows, care team roles and implementation best practices from clinic programs that address food insecurity and chronic conditions
- Consider ways to successfully collect data on food insecurity and related health outcome improvement programs despite EHR limitations
